List Requests
The NDBON makes available a nursing list containing data on LPNs, RNs and APRNs. These lists are updated on the second business day of each month (excluding state holidays). Please note that no refunds will be issued for list purchases and that licensee's home mailing address, telephone number, and email address are identified in N.D.C.C. as exempt data. The NDBON does not provide exempt data upon public request.
The cost for lists as are follows:
- LPN: $20
- RN (with APRN designations): $50
- APRN Only: $15.
LPN Lists will contain only the following fields in an excel spreadsheet file (.xlxs file):
- First Name
- Last Name
- License #
- Emp Practice Area
- LPN License Expiration Date
- LPN Initial Licensure Date
- LPN Compact Indicator (1 indicates a ND LPN Compact License; blank or zero indicates a ND Single State LPN License)
- Employer Name
- Employer Address
- Employer City
- Employer State
- Employer Zip
- Disciplinary Type (if they have ever had disciplinary action taken)
RN & APRN Lists will contain only the following fields in an excel spreadsheet file (.xlxs file):
- First Name
- Last Name
- License #
- Emp Practice Area
- RN License Expiration Date
- RN Initial Licensure Date
- RN Compact Indicator (1 indicates a ND RN Compact License; blank or zero indicates a ND Single State RN License)
- Employer Name
- Employer Address
- Employer City
- Employer State
- Employer Zip
- Type of APRN (NP=Nurse Practitioner; NC=Nurse Clinician, CNS=Clinical Nurse Specialist; CNM=Certified Nurse Midwife, CRNA=Certified Registered Nurse Anesthetist)
- Specialty Area
- APRN Expiration Date (if this date is prior to current date, their APRN is inactive; if it is blank they have never had an Advanced practice License)
- Initial APRN Licensure
- RX Expiration Date (if this date is prior to current date, their RX authority is inactive; if it is blank they have never had RX authority)
- Prescriptive Authority (RX) Initial License Date
- Disciplinary Type (if they have ever had disciplinary action taken)
- RN Inactive-APRN/SPRN Active (1 in this field indicates they are currently practicing in ND with a ND Advanced Practice license and an RN Compact License from another Compact State; a blank or zero indicates they have a ND RN license)
